Weird database error

I just made a comment on LJ using my bicycle icon:

Only what actually turned up, and turned out to be the icon associated with my "bike" keyword, necessitating me to re-upload the original one, was this, about which I know nothing:

Has anyone had anything like this happen to them on LJ? The two icons share a filename 1387433, but are in different directories in the URL (82930972 vs. 81598851).

It's a known issue, and it usually turns out to be a temporary glitch. Very mystifying bug, though.
